scheduled for one-hour time slots and follow a similar pattern to office
visits.

How phone consultation works
| Arrange an appointment time
with one of our doctors via e-mail or phone. |
| Download, print
out and fill out the appropriate intake forms for your first
phone consultation. Return to our office via mail or fax
(650-331-7250) prior to your phone consultation. |
| On arranged appointment time
and date the doctor will call you at your designated phone number. |
| During the first phone
consultation you will be discussing with the doctor the following:
| History of illness |
| Symptoms and duration of
illness |
| Aggravating and
alleviating factors |
| Current dietary intake
and lifestyle factors |
| and much, much more. |

| You will receive a
treatment plan and appropriate lab testing kits and recommendations
at the end of your consultation |
| Follow-up is usually in
3-4 weeks after first phone consultation |
Cancellation policy
| 24 hours notice required. |
| You will be
charged your scheduled visit time for missed
appointments or late cancellations. |
| A credit card deposit
will be required for securing you appointment time (your credit
card number is secure and will not be billed but will be
authorized) |
